オハイオ州トレドに住む12歳の少女リリーは、友達を家に呼びガールズトークに花を咲かせていると「血まみれ・メアリー」ゲームをする破目に。「血まみれ・メアリー」は古くからの言い伝えにちなんだ子供たちの肝だめし。友達に言われるままトイレの鏡の前で「血まみれ・メアリー」の名前を3回唱えるリリー。するとその直後にリリーの父シューメーカーがトイレで目から大量に血を流し死んでしまう。この事件に興味を持ったサムとディーンはさっそくトレドに向かった。
